Okay everyone,
I changed the link on the right to send you to a google site that is hosting our artist registration form. From there you can download it as a pdf or as a doc. Remember to open in a new tab so you don't leave the blog site!
Sorry about the mayhem, I hope everyone who needed the form in the meantime emailed me for it!
